一般社団法人非営利芸術活動団体コマンドN（以下、「当団体」といいます。）は、以下のように当団体のプライバシーポリシー（以下「本プライバシーポリシー」という）を定めます。
本プライバシーポリシーは、当団体が本ウェブサイト運営をはじめとした事業を行うにあたり、個人情報をどのような方針で利用・管理するかについて定めたものです。当団体は、当団体のサービスを安心してご利用いただくためには、個人情報保護についての法令を遵守し、適正な収集、利用、管理を社内において徹底することが最も重要であると考えています。

1. 用語の定義

本プライバシーポリシーにおいて「個人情報」とは、氏名、性別、メールアドレス、電話番号、住所、会社名、職業、生年月日、クレジットカード情報、その他連絡先等のうち、一つまたは複数の組み合わせにより、特定の個人を識別することができる情報をいいます。

当団体は、その事業を行うことに伴い、事業遂行上必要となる個人情報を取得しますが、これらの個人情報は以下の目的で利用します。また、当団体は、業務を円滑に進めるため、個人情報の取扱いの全部または一部を第三者に委託し、委託先に対して、必要な範囲で個人情報を提供することがありますが、この場合、当団体は、これらの委託先との間で個人情報の取扱いに関する契約の締結をはじめ、必要かつ適切な監督を行います。

3. 個人情報の管理について

当団体は、その取り扱う個人データの漏えい、滅失又はき損の防止その他の個人データの安全管理のために必要かつ適切な措置を講じます。又は当団体の事務員に個人データを取り扱わせるに当たって、当該個人データの安全管理が図られるよう、当該事務員に対する必要かつ適切な監督を行ないます。

4. 個人情報の共同利用について

当団体は、上記の「2．個人情報の利用について」に記載した業務委託先への提供の場合および以下のいずれかに該当する場合を除き、個人データを第三者へ開示または提供しません。

本人の同意がある場合
法令に基づく場合
人の生命、身体または財産の保護のために必要がある場合であって、本人の同意を得ることが困難であるとき
国の機関もしくは地方公共団体またはその委託を受けた者が法令の定める事務を遂行することに対して協力する必要がある場合であって、本人の同意を得ることにより当該事務の遂行に支障を及ぼす恐れがあるとき
本人を識別することができない状態で開示・提供する場合

5. 個人情報の提供について

当団体は、個人情報を適切に保護し、個人情報保護法その他の法令等により認められた場合を除き、個人識別が可能な状態で同意なく第三者に開示、提供することはありません。

6. 個人情報の開示・訂正・利用停止・消去等について

当団体は、個人情報保護法その他の法令等に基づき、本人が自己の個人情報について、開示、訂正、追加、削除、利用停止、消去、第三者提供の停止、利用目的の通知を求める権利を有していることを確認し、これらの要求がある場合には、当団体所定の手続きにより速やかに対応します。

7.個人情報開示請求等の受付方法について

個人情報の開示、訂正、追加、削除、利用停止、消去、第三者提供の停止を請求する場合は、当団体が別に定めるところに従い、所定の請求書に必要事項をご記入いただき、本人確認書類に当団体の個人情報相談窓口宛てに郵送して下さい。開示請求の場合は、所定の手数料・郵送料も同封してください。
